Seniors Outreach Services
Outreach is a core service of St. Aidan’s, providing assessments and personalized support plans to meet each senior’s unique needs. Using a case management approach, our team offers ongoing support and connects seniors with community resources to improve their quality of life.
 
    Assessment
We begin with an initial assessment. Through home visits and phone calls, we gather information to evaluate the senior’s current needs and connect them with proper programs and services. 
This assessment helps us collect detailed insights into a senior’s living situation—including health, mobility, transportation, finances, and challenges related to hearing, vision, or social connections. It also highlights their strengths and identifies areas where support may be needed.
Referrals
The assessment provides a comprehensive picture of each senior’s circumstances. This allows us to make tailored referrals that address their specific needs. All information is kept private and confidential. 
Services
Seniors Outreach Services can be delivered in the senior’s home, at our office, or within the community—whichever setting is most comfortable and convenient. 
Case Management
Our Outreach Workers provide ongoing support through case management, which is personalized to adapt to each senior’s evolving needs. Seniors are always in control and can choose the level and type of assistance they wish to receive.

With the senior’s consent, Outreach Workers may coordinate care by sharing relevant information with other professionals, support agencies, family members, or caregivers to ensure the most appropriate and cohesive support is provided.
Participation in the Seniors Outreach Program is completely voluntary. Support is offered only to those who wish to receive it, and seniors have the right to decline our services at any time.
