Information Sessions and Workshops at the Wood Buffalo Primary Care Network include:
Type I and II Adult Diabetes: A three hour group session that will help you learn how to control your diabetes with healthy eating and activity, manage your medication, and prevent other health conditions related to diabetes. Call the Primary Care Network to register for a session.
Weight Management: A group workshop which also includes one-on-one consultations. Individualized exercise plans will be created based on your initial assessment. Call the Primary Care Network for more information; however in order to enrol into the course a referral must be made by your doctor.
Heart and Stroke Program: Open to those who have a history of cardiovascular disease and those who may be at risk. Patients of the program have the choice of attending group education sessions or one-on-one visits with a wide variety of health professionals, including dietitians, a cardiac nurse, an exercise specialist, mental health therapist, and a pharmacist. A referral must be made by your family physician unless you have recently been recently hospitalized due to a cardiac event; in which a registered nurse will make a referral.
Secondary Stroke Prevention Clinic: Provides individualized one-on-one care to those who are recovering from a stroke. A registered nurse will focus on risk reduction and prevention strategies. Referrals to the Stroke Clinic are made either by a patient’s family physician, or the Stroke Navigator at the Fort McMurray Hospital.
Lung Wellness – Pulmonary Rehabilitation: Provides patients with chronic lung problems the ability to better manage their disease through clinically-supervised exercise and education services. Ask your local family physician if you would benefit from participation. The program duration is 8 weeks with two sessions per week (Tuesday’s, and Thursday’s). Call the Primary Care Network for more information or have your family physician send a referral in.
Lung Wellness – Smoking Cessation: Offers free smoking cessation classes with no referral needed. Participants are offered stress-free appointments with a pharmacist, who will explain the quitting process and prescribe smoking cessation medication to help you along your way.
