The Home Care/Continuing Care Social Worker provides a range of generalist social work services to clients and their families, including, but not limited to:

Assessing client/family resource based needs related to finances, housing, transportation, employment, immigration and diversity services, and other relevant resources.
Referral to specialized resources and programs (diabetic programs, addictions treatment and mental health counselling services).

Psychosocial support and crisis intervention to clients and families during periods of crisis, vulnerability, and transition.
Supportive and adjustment counselling during periods of bereavement, new medical diagnosis, adjustment to chronic illness, caregiver support, end of life planning, coping, family stress, and life transitions.

Advocate for client/family centered care, and resources.
Liaise/collaborate with community supports to enhance client/family care.
