Ages 65-74 – $29/Month, Ages 75-84 – $31/Month, Ages 85 and up – $30/Month

Includes: Semi-private and private hospital rooms, $20/visit to a Chiropractor to a maximum of $150 each benefit year, 65% basic dental coverage (up to $500 each benefit year), 70% vision care coverage (up to $150 toward the purchase and repair of eyeglasses or contact lenses once every three years), and $10,000 in the event of an accidental death.

Seniors Plus Plan B:

Ages 65-84 – $42/Month, Ages 85 and up – $40/Month

Includes: Semi-private and private hospital rooms, $20/visit to a Chiropractor to a maximum of $250 each benefit year, 70% basic dental coverage (up to $500 each benefit year), 65% extensive dental coverage (combined basic and extensive dental maximum of $850 each benefit year), 70% vision care coverage (up to $200 toward the purchase and repair of eyeglasses or contact lenses once every three years), and $15,000 in the event of an accidental death.

Ages 65-74 – $55/Month, Ages 75-84 – $56/Month, Ages 85 and up – $55/Month

Includes: Semi-private and private hospital rooms, $25/visit to a Chiropractor to a maximum of $300 each benefit year, 70% basic dental coverage (up to $600 each benefit year), 65% extensive dental coverage (combined basic and extensive dental maximum of $1,100 each benefit year), 70% vision care coverage (up to $250 toward the purchase and repair of eyeglasses or contact lenses once every three years), and $20,000 in the event of an accidental death.
